Tri-lingual German, Ukranian, and Polish questionnaire to be completed for the German authorities by health professionals reporting for the first time (Fragebogen zur erstmaligen Meldung der Heilberufe). The form, on green paper, is comprised of thirty-four questions. The questionnaire is to be completed by doctors, druggists, dentists, and others. This copy is stamped with a Jewish star, the individual completing the form being Arnold (Abraham Jacob Tauber, a Jew. His photograph is attached to the form. It begins with family name, (woman provide maiden name), personal name, address, place of practice. It inquires as to marital status, Tauber is unmarried, wife’s maiden name (not completed as inapplicable), residence as of 1939 (Poland), genalogical information such as grandparents’ names, Dr. Tauber lists all four family names, among them:
Tannembaum, Bettman; professional background such as education and training; experience in World War I; whether having served in the Polish army; whether a specialist, profession (he is a dentist); and many others, not all of which were applicable to or answered by Dr. Tauber. It is not known whether Dr. Tauber survived the war.
