Paragraph 2    די... תפלה... איז פון די... צדיקים זצ"ל, און פון די מקובלים פון ארץ ישראל... פאר אריכת ימים ...אויך געגען שלעכטע חלומות ועין הרע... פאר א קראנקען... ([מאת] יעקב הלברשטאם האבד"ק טשאקאווע).

במעטפת: תפלה צו בעטען פאר געזונד... בראש הספר הקדמה מאת ר' יעקב האלברשטאם, האדמו"ר מטשאקוב, הכותב שהוא חיבר את התפילה וסדר אמירת הפסוקים וכו'. התפילה ביידיש. שאר הדברים בעברית. יידיש ואנגלית. נדפס על נייר צהוב משובח בדיו כחולה.
